We would like to thank you for taking part in our process of submitting athletic participation paperwork online. The automated process allows us to be more efficient in handling the forms as well as saving valuable resources. All forms will be submitted in this manner except for the ImPACT Test. The UIL Pre-Participation Physical and Medical History may be submitted online or as a hard copy. INSTRUCTIONS: UIL PRE-PARTICIPATION PHYSICAL & MEDICAL HISTORY CLICK THE LINK BELOW AND FILL OUT THE MEDICAL HISTORY (SIDE 1) AND THE TOP LINE ONLY OF THE PHYSICAL (SIDE 2). PRINT. SIGN THE BOTTOM OF SIDE 1. TAKE TO THE DOCTOR. https://www.uiltexas.org/files/athletics/forms/PrePhysFormRvsd2024.pdf *Note for physicals: All physicals should be completed on the current up-to-date UIL Pre-Participation Medical History and Physical Form (2024) for the 2024-25 school year dated not prior to May 1, 2024. Make a copy for yourself. Then either give the original to the specified contact on your child’s campus (usually a head coach or athletic trainer) OR upload directly in the RankOne database under Physical Upload Form. INSTRUCTIONS: FISD & UIL PRE-PARTICIPATION FORMS ATHLETE AND PARENT MUST SIGN ALL FORMS!! You must have your students’ Frisco ISD 6-digit student ID number and it must accurately match the LAST name of the athlete that the forms are being submitted for. The forms cannot be submitted if these do not match. There are 3 forms you may submit electronically and 2 paper documents with alternate submission requirements. To access the online forms: 1. Click on Proceed to Online Forms” tab at the bottom of this screen. 2. Either log in to your parent account OR you may click on Continue as a guest”. 3. Scroll to the Electronic Documents” tab and complete the forms. -a. Click on the form name and fill out the information requested. -b. To sign the document, click inside the signature box and hold your mouse down. This will allow you to create an Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box. -c. Once you have filled out all of the information on each page you will have the opportunity to print the document. This is NOT required and would be for YOUR use only. -d. Enter a valid email address at the bottom of the form and you will receive a confirmation email once the document has been submitted and received by the RankOne system. To submit the Physical/Medical History form choose 1 of the following 2 options: 1. Make a copy for your records and submit the original to your campus athletic trainer or designated coach. 2. Submit electronically. -a. Scroll to the Electronic Documents” tab -b. Click on Physical Upload Form -c. Scan/photograph both sides of the form separately. -d. Upload as directed.
