This new automated process will allow us to be more efficient in handling the forms as well as saving valuable resources. All forms will be submitted in this manner except for the UIL Pre-participation Physical and Medical History. INSTRUCTIONS: UIL PREPARTICIPATION PHYSICAL & MEDICAL HISTORY CLICK THE LINK BELOW AND FILL OUT OR PRINT FOR THE DOCTOR Click here to view the Physical •Note for physicals: All physicals should be completed on the current up-to-date UIL Pre-participation Medical History and Physical Form (2019) for 2019/20 school year not prior to May 1, 2019 click link below for the form. Print the forms fill out the Medical History form and take them to your doctor at the time of the physical exam. INSTRUCTIONS: FISD & UIL PREPARTICIPATION FORMS: Athlete and Parent must sign all forms!!!! You must have your students ID number and it must accurately match the last name of the athlete that the forms are being submitted. The forms can’t be submitted if these do not match. To access the online forms click on the “Electronic Participation Forms” tab. There are 3 forms you may submit electronically, you must print the Pre-participation & Medical History Forms and take them to the Dr.’s office on the date you get the physical examination. • Click on the form name and fill out the information requested. • To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box. • Once you have filled out all of the information on each page you will have the opportunity to print the document. • Enter a valid email address at the bottom of the form and you will receive a confirmation email once the document has been submitted and received by the RankOne system. – If you have any questions please contact the athletic trainer at your high school or athletic coordinator at your middle school. © 2007 - 2020 Rank One Sport | All rights reserved.
